My Child, has permission to participate in the High School Open House Day trip to Tuskegee University, Tuskegee, Alabama sponsored by the Augusta Tuskegee University Alumni Club on Saturday, September 23rd, 2023.  I agree to indemnify the Augusta Tuskegee Alumni Club and hold the Augusta Tuskegee Alumni Club harmless from any liability, claims, demands, and judgments arising at any time when I and /or my child participate in any program activity.  I am voluntarily participating in the above-mentioned activity with the awareness of the risks associated with Covid-19.  I further understand that the Augusta Tuskegee Alumni Club does not provide participants' insurance, nor does it assume responsibility for such accidents or injuries.

 

I authorize an Augusta Tuskegee Alumni Club chaperone associated with this event to act on my behalf to authorize medical treatment, upon, or for the benefit of myself and/or my minor child, for any minor injuries that may occur from our participation in the Tuskegee University Fall Open House Day and associated activities.  I recognize that such treatment shall be my full responsibility. If a more serious injury may require emergency treatment, I authorize such personnel to see myself and/or my minor child transported to and treated at the nearest medical facility, with the related expenses being my full responsibility.

I understand that the Tuskegee Open House T-shirt must be worn all day.  Students not wearing the T-Shirt will not be allowed to board the bus.  No refunds will be issued to students who miss the bus.  Masks are to be worn while on the bus or when social distancing is not possible.
